Rename the file by clicking on it with the mouse or hitting the F2 key, from index.html to index.php
To create a subfolder, there is a button to do this above each file listing. If you run over the buttons with your mouse you will see a button "Make Directory". There is also an option by right clicking, under "Directory Commands".
